Job Overview
We are seeking a dedicated OVERNIGHT LPN to join our clinical team, specializing in high-acuity care within a Long-Term Residential Skilled Nursing GROUP HOME setting. This role focuses on supporting a small, consistent group of up to seven individuals with developmental disabilities or dementia. The ideal candidate brings strong clinical judgment and specialized expertise in Tracheal-intubation care and Therap Software documentation. SHIFT HOURS: 11PM-7AM. LOCATIONS: 30316 & 30032.
Responsibilities
To include but not limited to
- Specialized Clinical Care: provide tracheal intubation care, manage airway devices (including ventilators), and perform complex procedures such as catheterization, nephrostomy care, and GJ Tube (gastrostomy) feeding.
- Patient Assessment: Conduct comprehensive physical exams, monitor vital signs, and update healthcare screenings to ensure proactive intervention.
- Medication & Therapy: Administer medications and injections; manage CP Therapy (Vest) and provide sterile specimen collection.
- Documentation & Compliance: Maintain precise electronic records in Therap while strictly adhering to HIPAA and infection control protocols.
- Care Coordination: Manage discharge planning, facilitate case management, and provide health coaching on chronic disease management, when warranted.
- Patient Advocacy: Escort residents to diagnostic appointments and collaborate with multidisciplinary teams to ensure holistic care, when warranted.
Qualifications
- Licensure: Valid LPN or RN license in the state of practice; BLS Certification through the American Heart Association is highly preferred, but we will accept other vendors.
- Clinical Expertise: Proven experience with Tracheal Care, sterile processing, and diagnostic evaluations. Familiarity with ICD-10/CPT coding is a plus.
- Specialized Experience: Prior work in Behavioral Health, Hospice, or with the developmentally disabled population is advantageous.
- Soft Skills: Exceptional communication and the ability to thrive in a very slow-paced, high-attention-to-detail residential environment.
- Compliance: Ability to pass a background check and drug screen; must provide a negative TB result from within the last 6 months.
