Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using paper documentation.
- Administer medications and injections accurately following physician orders while adhering to HIPAA regulations and sterile techniques.
- Manage IV infusions, catheterizations, tube feedings, and ventilator care with precision and attention to infection control protocols.
- Conduct diagnostic evaluations such as blood draws (phlebotomy), specimen collection/processing, and basic laboratory procedures.
- Provide wound care, suture removal, and skin care when necessary.
- Support emergency response efforts by managing airway obstructions, ventilator settings, or acute pain episodes in collaboration with medical teams.
- Assist with patient mobility using equipment like Hoyer lifts and ensure safety during transfers or activities of daily living.
- Document all patient interactions thoroughly in medical records, including medication administration, patient assessments, and discharge planning.
- Collaborate with multidisciplinary teams on case management plans for patients with complex needs such as dementia care, Alzheimer's disease, or behavioral health issues.
- Participate in infection control practices and maintain compliance with NCQA standards for quality care delivery across inpatient and outpatient settings.
Qualifications
- Valid state licensure as a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN).
- Proven experience in diverse healthcare environments such as hospitals (including ICU, emergency room, trauma centers), nursing homes, hospice care, or home & community care settings.
- Strong knowledge of medical terminology, anatomy & physiology, ICD coding (ICD-9/ICD-10), CPT coding, and medical documentation standards.
- Experience administering injections, IV insertion/infusion therapy, catheterization, suturing, and tube feeding is highly preferred.
- Familiarity with standard protocols to ensure accurate documentation.
- Ability to perform physical examinations and diagnostic evaluations confidently while demonstrating excellent patient assessment skills.
- Knowledge of infectious disease protocols and infection control measures within acute care or long-term care environments.
- Exceptional communication skills to work effectively with patients of all ages—including pediatrics and neonatal ventilator management—and collaborate seamlessly with healthcare teams.
- Demonstrated ability to work with people with disabilities or developmental challenges in assisted living or home care settings.
