Licensed Practical Nurse

Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. Duties

  • Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to inform care plans.
  • Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management.
  • Assist with diagnostic evaluations such as specimen collection, processing, and medical imaging procedures.
  • Provide specialized care for patients with complex needs such as Alzheimer’s disease, dementia, traumatic injuries, or infectious diseases.
  • Support emergency situations by managing airway control, ventilator support, acute pain management, and triage in urgent or hospital settings.
  • Document all patient interactions thoroughly using EMR (Electronic Medical Records) systems like Epic or Cerner while maintaining HIPAA compliance.
  • Collaborate with multidisciplinary teams on discharge planning, case management, and patient education to ensure seamless continuity of care.

Requirements

  • Valid state licensure as a Licensed Practical Nurse (LPN) with current certification.
  • Proven experience in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, or outpatient clinics.
  • Demonstrated proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks.
  • Critical care experience preferred; familiarity with ICU procedures like ventilator management and spinal taps is a plus.
  • Knowledge of ICD-9/ICD-10 coding for accurate documentation and billing processes.
  • Strong understanding of medical terminology, anatomy, physiology, and pharmacology relevant to patient care.
  • Ability to perform advanced nursing skills such as IV insertion, catheterization, sterile technique, phlebotomy, suturing, and tube feeding.
  • Excellent patient assessment skills with the ability to work with individuals across age groups including pediatrics and geriatrics.
  • Exceptional communication skills for effective triage and health coaching; capable of working independently or within a team environment.
