Licensed Practical Nurse

Job Overview
We are seeking a Home Health Nurse that is compassionate and dedicated to join our healthcare team. The ideal candidate will be responsible for providing high-quality patient care in the clients homes. The LPN will work closely with registered nurses and other healthcare professionals to ensure the well-being of patients while adhering to established protocols and standards.

Responsibilities

  • Deliver direct patient care, wound care, tracheostomy's care and administering medications and treatments as prescribed.
  • Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
  • Assist in the development and implementation of individualized care plans.
  • Provide support for patients with developmental disabilities, Alzheimer's, dementia, and other memory care needs.
  • Perform IV infusion therapy and catheterization as required.
  • Maintain accurate medical documentation in compliance with HIPAA regulations.
  • Collaborate with interdisciplinary teams to coordinate patient care and discharge planning.
  • Educate patients and their families about health management, medication administration, and treatment options.
  • Participate in infection control measures and maintain a clean, safe environment for patients.

Requirements

  • Current state licensure as a Licensed Practical Nurse (LPN).
  • Experience in critical care, long-term care, or assisted living preferred.
  • Familiarity with EMR/EHR systems such as Epic, Cerner, or Athenahealth is a plus.
  • Knowledge of medical terminology, ICD coding (ICD-9/ICD-10), and CPT coding is beneficial.
  • Proficiency in patient assessment techniques and basic life support (BLS).
  • Experience in specialized areas such as pediatrics, geriatrics, hospice care, or emergency medicine is advantageous.
  • Strong communication skills and the ability to work collaboratively within a healthcare team.
  • Compassionate attitude towards patients with varying needs including those requiring acute pain management or post-operative care.
