DUTIES AND RESPONSIBILITIES:
· Collaborate with companies that offer incentives to providers for achieving specific clinical initiative targets while improving overall patient care delivery.
· Participate in programs to enhance patient quality of care.
· Attend required meetings and participate in committees as assigned.
· Work with the current support associations and companies when needed.
· Serve as the contact point, advocate, and informational resource for patients, care team, family/caregivers, payers, and community resources.
· Assist with the identification of "high risk" patients (the chronically ill) and those with special healthcare needs.
· Provide feedback for the improvement of the Care Coordination Program.
· Encourage patients to participate in preventative health care screenings.
· Create and promote adherence to a care plan, developed in coordination with the patient, primary care provider, and family/caregiver(s).
· Provides medication reconciliation.
· Connect patients to relevant community resources with the goal of enhancing patient health and well-being, increasing patient satisfaction, and reducing health care costs.
· Working knowledge of current healthcare practices and clinical theory.
· Accurate documentation of all patient information.
· Follows standards for Universal Precautions.
· Provide patient education.
· Serve as patient advocate.
· Other duties as assigned by supervisor.
QUALIFICATIONS:
· Ability to communicate effectively orally and in writing.
· BCLS required.
· Computer literate, detail oriented, flexible personality.
· Graduate of accredited school of nursing.
· Ability to understand that unauthorized discussion and/or disclosure of protected health information in any form, either on or off the premises of Selma Medical Associates, Inc., is strictly forbidden.
· Understand that the Privacy and Security of medical records is covered by the Privacy Act of 1974 and Federal Regulation 45CFR, Parts 160, 162, and 164 Standards for Privacy and Security of Individually Identifiable Health Information.
· Must have a working knowledge of current healthcare practices and nursing theory. Oral and written communication skills. Skilled in patient assessment and problem solving. Ability to interpret and follow current guidelines and policies. Skilled in organization and coordinating patient care.
· Good logical and analytical skills.
· Must be able to respond to others, patients, and co-workers in a caring, calm, and supportive manner.
· Detail oriented, able to multi-task and remain flexible with assignments.
