Duties
- Assume responsibility for the overall management of nursing services during your shift, ensuring smooth operations and adherence to clinical standards.
- Supervise and support nursing staff, fostering a collaborative environment focused on delivering exceptional patient care.
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to inform treatment plans.
- Administer medications, injections, IV infusions, and perform catheterizations following strict protocols and safety guidelines.
- Coordinate interdisciplinary care plans, including discharge planning, case management, and collaboration with physicians across specialties such as geriatrics, emergency medicine, ICU, and hospice care.
- Maintain accurate medical documentation using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Point Click Care.
- Lead infection control efforts, monitor for infectious disease risks, and implement aseptic techniques to safeguard patient health.
- Provide education on medication administration, pain management strategies—including acute pain management—and health coaching tailored to diverse patient populations.
- Oversee triage procedures for urgent cases and participate in diagnostic evaluations utilizing medical imaging or laboratory testing when necessary.
- Ensure all nursing activities align with NCQA standards and adhere to state-specific licensing requirements such as ICD-10/ICD-9 coding for documentation accuracy.
Qualifications
- Valid licensure as a Licensed Practical Nurse (LPN) with current state certification.
- Proven experience in critical care environments such as ICU, emergency room, or Level I/II trauma centers; hospital medicine experience preferred.
- Demonstrated expertise in medical-surgical nursing, geriatrics—including dementia and Alzheimer’s care—and long-term care settings like assisted living or nursing homes.
- Proficiency with EMR/EHR systems such as Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with medical terminology and ICD coding is essential.
- Strong knowledge of physiology, anatomy, pharmacology related to medication administration—including injections—and tube feeding procedures.
- Experience working with individuals with disabilities or developmental disabilities; familiarity with behavioral health or pediatric care is advantageous.
- Ability to perform venipuncture, phlebotomy, sterile processing techniques, suturing, airway management—including ventilator use—and other advanced clinical skills.
- Excellent communication skills for effective patient assessment and interdisciplinary collaboration; leadership qualities to serve as a charge nurse during shifts.
