Licensed Practical Nurse - Charge Nurse

Job Summary
We are seeking a dynamic and compassionate Licensed Practical Nurse (LPN) to step into the vital role of Charge Nurse within our healthcare team. Duties

• Provide direct patient care with a focus on safety, comfort, and clinical accuracy, including vital signs assessment and medication administration.
• Supervise and support nursing staff during shifts, ensuring adherence to care plans and regulatory standards.
• Coordinate patient assessments, develop individualized care plans, and monitor progress through thorough documentation.
• Oversee the management of medical records using EMR (Electronic Medical Record) systems such as Epic or Cerner, ensuring accuracy and compliance with HIPAA regulations.
• Lead emergency response efforts when necessary, including airway management, IV infusion, and acute pain management protocols.
• Facilitate communication between multidisciplinary teams, patients’ families, and external healthcare providers to ensure seamless care transitions.
• Conduct staff training on infection control practices, sterile techniques, and proper use of medical equipment such as ventilators or catheters.

Skills

• Proven experience with EHR platforms is a plus.
• Strong knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation methods.
• Skilled in medication administration including injections, IV insertion/maintenance, and catheter care.
• Ability to perform comprehensive patient assessments and physical examinations accurately.
• Experience working with diverse populations including seniors in assisted living or long-term care environments; familiarity with dementia and Alzheimer’s care is advantageous.
• Proficient in infection control procedures, tube feeding management, and basic life support (BLS).
• Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures like suturing or spinal taps is desirable.
• Capable of triage assessment in urgent or emergency situations; experience in urgent care or hospital medicine preferred.
• Excellent communication skills for effective case management and patient education; experience working with individuals with disabilities or developmental challenges is beneficial.
