Licensed Practical Nurse - Charge Nurse M-F - 3-11pm

Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in our skilled nursing facility. As an LPN, you will play a crucial role in supporting the health and well-being of residents while working collaboratively with other healthcare professionals.

Responsibilities

• Provide direct patient care, including administering medications and treatments as prescribed by physicians.
• Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
• Assist with daily living activities for patients, including bathing, dressing, and mobility support.
• Maintain accurate medical records and documentation in compliance with HIPAA regulations.
• Collaborate with healthcare teams to develop and implement individualized care plans.
• Educate patients and their families on health management, medication administration, and disease prevention.
• Support patients with chronic conditions such as Alzheimer’s disease, dementia, and other developmental disabilities.
• Participate in discharge planning and coordinate post-acute care services.

Experience
Candidates should have experience in one or more of the following areas:

• Long-term care or assisted living environments
• Home care or hospice care
• Experience working with individuals with developmental disabilities
• Familiarity with EMR/EHR systems such as Point Click Care.
• Experience in geriatrics
• Additional skills that are beneficial include:
• Patient assessment techniques
• Basic life support (BLS) certification
• Experience administering injections
• Knowledge of infection control practices

Pay: From $28.80 per hour
