Licensed Practical Nurse (LPN)

Job Summary

The Licensed Practical Nurse (LPN) is responsible to the Director of Nursing and provides basic nursing care under the supervision of a Registered Nurse and/or Medical Provider. The LPN collaborates with clinical, clerical, administrative, and support staff to deliver comprehensive, patient-centered care in a high-intensity outpatient setting.

Essential Duties and Responsibilities

  • Deliver direct patient care in accordance with the scope of practice, clinical policies, and provider orders.
  • Administer medications (oral, injection, topical) and perform venipuncture as directed by the provider.
  • Collect and document patient vital signs and health history (pulse, temperature, blood pressure, weight, height).
  • Assist with clinical documentation and ensure patient records are complete and up to date.
  • Communicate effectively with patients, providers, and healthcare team members.
  • Participate in patient triage, screening, and care coordination activities.
  • Support and participate in quality improvement and infection control initiatives.
  • Educate patients and families regarding treatment plans and follow-up care as directed.
  • Assist in resolving patient complaints related to clinical services and contribute to service recovery.
  • Maintain clinical competencies and stay current on best practices through professional development.

Team and Administrative Responsibilities

  • Promote a team-based approach to care delivery; assist colleagues in completing tasks as needed.
  • Participate in staff meetings, required corporate trainings, and committee activities as requested.
  • Assist with the orientation and training of new clinical employees.
  • Provide input in hiring decisions and staffing schedules to support efficient clinic operations.
  • Assist the DON and/or CMO in ensuring compliance with standards related to grants, quality improvement, legal, and regulatory requirements.
  • Help maintain and update clinical procedure manuals, standing orders, and clinical indicators.
  • Participate in internal audits of clinical operations, medical records, and emergency equipment.
  • Assist in monitoring nursing and medical assistant competencies annually.
  • Coordinate or support in-service education and staff immunization updates (VFC, GRITS, etc.).
  • Contribute to accreditation processes and other organizational initiatives as needed.
  • Support strategic planning efforts aligned with the mission and goals of Neighborhood Improvement Project, Inc.
  • Perform other duties as assigned by the CEO, DON, or CMO.

Case Management and Vaccine Management

  • Provide case management services as directed.
  • Ensure proper management and documentation of vaccines; maintain provider profile agreement and GRITS reporting.
  • Coordinate and deliver clinical education regarding immunizations and related updates.

Typical Physical Demands

  • Requires full range of body motion including standing, walking, bending, lifting, and handling patients.
  • Must be able to lift and carry up to 50 pounds occasionally.
  • Manual dexterity and eye-hand coordination required.
  • Requires corrected vision and hearing within normal range.
  • Frequent exposure to communicable diseases, bloodborne pathogens, and bodily fluids.
  • Must be able to work under stress and in fast-paced environments.

EDUCATION:

  • Graduate of a program for Nursing with a minimum of ASN.
  • Current licensure as a registered LPN in Georgia
  • A minimum of two years’ experience in leadership of clinical services provision and continuous quality improvement.

Working Conditions

This position requires regular interaction with patients, providers, and support staff. Professionalism, tact, and sound judgment are necessary. Occasional after-hours or weekend work may be required based on operational needs.
