Licensed Practical Nurse (LPN)

JOB DESCRIPTION: LPN - REMOTE/HYBRID

Job Description:

Proven Home Healthcare Agency LPN will administer medication as necessary and create an ongoing report on the patient's progress. Our nurse's responsibilities include working with other healthcare professionals to create treatment plans, ensuring that patients stick to treatment plans, and monitoring patients' vital signs.

Our home health nurse possesses patience as well as empathy towards our clients and their families.

Responsibilities:

· Develop a care plan for each new client and do supervision visits as needed.

Administering medication to clients as assigned / as needed.

• Creating monthly reports on the Portal as needed to track the client's progress based on caregivers' daily reports on the Provens portal.
• Working with other professionals to create treatment plans.
• Cleaning and re-dressing wounds if needed.
• Measuring vital signs as needed.
• Monitoring changes in behavior and progress from caregivers' reports on portal and visits.

Requirements:

· At a minimum, an associate degree in nursing (ADN)

· Pass the NCLEX exam.

· Patience.

· Empathy.

· Flexible work hours.

· Ability to remain calm under pressure.
