THE ROLE

This position supports clients enrolled in Georgia's EDWP, SOURCE, and CCSP Medicaid Waiver programs — some of the state's most impactful long-term care programs.

Responsibilities:

• Conduct face-to-face nursing assessments in client homes, hospitals, or care facilities across Metro Atlanta
• Analyze client health data and ensure documentation meets program standards
• Collaborate with RNs, physicians, and case managers on care plan development and reviews
• Maintain clear communication with supervisors and participate in required trainings

QUALIFICATIONS

• Active Georgia LPN license in good standing
• Experience in geriatric care, community health, long-term care, or chronic disease management
• Highly self-directed — you don't need a manager standing over your shoulder
• Strong communicator with elderly clients, families, and care teams
• Compassionate, professional, and invested in the people you serve
