The Association for Individual Development (AID) is a non-profit organization whose mission is to empower people with physical, developmental, intellectual, mental health challenges; those who have suffered a trauma; and those at risk, to enjoy lives of dignity and purpose. We are looking for a Licensed Practical Nurse LPN who demonstrates this mission and wants to work for an organization that makes a difference in the community.
What you will be doing?
  • The Licensed Practical Nurse is responsible for the provision of part-time nursing services, under the supervision of a Registered Nurse, to meet the identified health care needs of a designated population of clients.
  • Responsibilities include assessing the health needs of clients, planning for the provision of needed health services, implementing a plan of care, and evaluating outcomes and ongoing health needs under the direction of a Registered Nurse.
  • The Licensed Practical Nurse collaborates with physicians, other health care providers, and the Service Planning Team, as needed, to address client health needs
  • The Licensed Practical Nurse, under the supervision of a Registered Nurse, works within the framework of AID’s goals, policies, and procedures to assure that accreditation standards and regulatory requirements about health care are met in all programs.
Essential Job Responsibilities
  • Under the direction of the Registered Nurse, completes timely assessment(s) of the client’s health, using various screening tools. (i.e., TD screening, Depression, PHQ9, etc.), as needed.
  • Enters all required assessments and other clinical documentation into the computer (CX360) promptly, according to program requirements, as needed.
  • Collaborates with client and RN, as needed, to assist in planning appropriate health services for clients, based upon client needs.
  • Participates in staff meetings, providing input and ideas for improvements when available.
  • Demonstrates consistent attendance and punctuality to scheduled work hours.
Implementation Skills
  • Collaborates with physicians, as needed.
  • Under the supervision of a Registered Nurse, implements physician orders and/or facilitates the implementation of physician orders.
  • Under the supervision of a Registered Nurse, provides medication administration, monitoring, and/or training for clients, as needed.
  • Promotes client wellness through coaching, counseling, consultation, motivational enhancement, and information and referral services to clients facing various health challenges, as needed
  • Communicate with service delivery partners, which may include Physicians, Nurse Practitioners, Counselors, Pharmacists, Employers, Community Supports, and other health professionals, as needed, to facilitate the achievement of the client’s wellness objectives.
  • Utilizes a variety of appropriate resources and educational materials for wellness coaching. (ie, Health Matters curriculum, Smoking cessation materials, Weight Management information, etc.), as needed.
  • Complete required nursing documentation promptly, recording at least 50% of time in direct client services, according to program requirements.
  • Assures the nursing department’s compliance with regulatory requirements and accreditation standards within the designated program.
  • Maintains client files according to AID procedures.
  • Provides consultation on community health resources, as needed, to address client health needs.
  • Maintains client confidentiality by agency policy and federal and state laws.
  • Acquire and maintain certification in CPR, non-violent crisis intervention (CPI), OIG Rule 50 training, and other mandatory or assigned trainings.
Evaluation Skills
  • In collaboration with a Registered Nurse, evaluates outcomes of nursing interventions, and adjusts plans, as needed.
  • Evaluates the safety of the work environment and takes corrective actions when needed or makes recommendations for safety improvements.
  • Broadens knowledge through seminars, conferences, formal education, and/or self-directed learning.
Additional Job Responsibilities:
  • Assesses changes within programs or the health care environment that may warrant the development of new health-related policies or procedures and reports these ideas to the Nurse Manager and/or Director of Health Services.
  • Assesses the need for client, guardian, or staff education on health-related topics, and acts to address these needs, as appropriate, or discusses these needs with the Director of Health Services to plan necessary training.
Planning Skills
  • Collaborates with Program Directors and/or Director of Health Services to improve client services or resolve problems.
  • Under the direction of a Registered Nurse, assists in the development, review, and updating of health care procedures, based upon client and/or agency needs.
  • Assists with the provision of medical and first aid supplies for programs and clients.
  • Under the direction of a Registered Nurse, assists with planning and updating selected health training curriculum (ie, medication training modules, Health and Safety modules, etc.), as needed.
Implementation Skills
  • Under the supervision of a Registered Nurse, provides client health teaching and/or health education for families, guardians, and staff, as needed.
  • Consults with and serves as liaison between the client, program, physician, and area hospital(s), or other agencies, as needed.
  • Facilitates the operation of the agency's psychiatric clinics, as needed.
  • Administers First Aid, as needed.
  • Assists in the orientation of new health services staff (nurses and dietitians) to their role.
Evaluation Skills
  • Collaborates with RN and treatment team to update and revise the client’s health-related or medication-related goals on IP, as needed.
  • Solicits input from customers regarding ways to improve the quality of services.
  • Assists in the Quality Assurance process, as needed.
  • Participates in agency committees, as needed.

What will you bring to the table?
Education: Must be a graduate of an accredited school of nursing.
License/certifications: Current Illinois State LPN License
Experience: One year of previous experience working with persons with developmental disabilities, mental illness, and/or neurological impairments are preferred.
Physical: Demonstrates the physical capacity to perform nursing responsibilities, including, but not limited to, pushing, pulling, reaching, bending, and physically assisting clients. Must be able to climb stairs and lift up to 50 pounds.
Equipment: Computer, Copy Machine, Fax machine, Telephone with voice mail system, Scales (Electric and Manual), Audio-visual equipment, Hoyer lift, Stethoscope, Blood Pressure Equipment, Technology, Electronic Health Records, AI programs as required.
Additional Requirements:
  • Must acquire and maintain certifications in CPR, Non-violent crisis intervention training, OIG- Rule 50, and attend other relevant trainings, as required
  • Position requires the ability to work a flexible schedule, which may include evening responsibility for the assigned program
  • Use of an employee's automobile for local and regional community travel between program sites. Drives self for agency business.
  • Valid Driver’s license Minimum amount of automobile liability insurance required, as defined by AID’s personnel policies.
  • Effective communication and interpersonal skills are required.
