Job Summary:

The LPN Home Care Administrator is responsible for overseeing and coordinating the day-to-day operations of a non-medical home care and DDD program. This position ensures that services provided to individuals with developmental disabilities and clients receiving personal care are delivered in accordance with state regulations, agency policies, and person-centered care principles. The LPN Administrator provides leadership, clinical oversight, and administrative support to ensure quality, compliance, and client satisfaction.

Key Responsibilities:

Oversee daily operations of the home care and DDD programs to ensure compliance with state regulations and agency standards.
  • Supervise and support Direct Support Professionals (DSPs), Home Health Aides (HHAs), and other care staff to ensure the delivery of safe, person-centered services.
  • Participate in the development, review, and monitoring of Individual Service Plans (ISPs) and care plans.
  • Conduct client visits and assessments as required to monitor health status, care quality, and service satisfaction.
  • Ensure timely and accurate documentation of services, progress notes, incidents, and communication logs.
  • Serve as the primary contact for families, guardians, case managers, and DDD representatives.
  • Participate in quality assurance and compliance reviews, ensuring corrective actions are implemented as needed.
  • Assist with hiring, onboarding, and training staff to meet agency and DDD standards.
  • Monitor staff schedules, attendance, and service delivery to ensure appropriate coverage and continuity of care.
  • Support health and safety initiatives, including medication management oversight and infection control practices.
  • Maintain client confidentiality and adhere to HIPAA and DDD compliance requirements.
  • Collaborate with the Agency Director on policy updates, performance goals, and program improvements.
Qualifications:

  • Current and active Licensed Practical Nurse (LPN) license in the State of [Insert State].
  • Experience in a leadership, supervisory, or administrative role preferred.
  • Working knowledge of DDD regulations, Medicaid requirements, and non-medical home care compliance.
  • Strong leadership, communication, and organizational skills.
  • Compassionate, patient, and professional approach when working with clients and staff.
  • Ability to handle confidential and sensitive information with integrity.
  • Proficiency in electronic documentation systems and Microsoft Office Suite.
  • Must have a valid driver’s license and reliable transportation for community visits.
