Licensed Practical/Vocational Nurse - Transfer Center

Coho Integrated Solutions is seeking a qualified Licensed Practical/Vocational Nurse - Transfer Center to perform professional nursing services in support of patient care operations at the Military Treatment Facility (MTF) in Camp Lejeune, NC. Licensed Practical/Vocational Nurse shall perform duties consistent with the standards of professional nursing practice, applicable federal and state regulations, and the policies and procedures of the MTF.

Responsibilities (include but are not limited to):

  • Coordinate with designated Emergency Department Staff to provide Ambulance transport for the patient and the Air Transport Team to and from helicopter landing pad and assist with the moving of the patient to the appropriate transport team.
  • Facilitate the transfer of emergent and non-emergent ER, ward, and clinic patients out to local and regional civilian or military facilities.
  • Serve as a central point of contact for patients transferred from and to NMCCL to external medical facilities (civilian or military); providing professional communication with patient/family members, healthcare workers (internal and external), military unit Medical Officers, MTF Directors, and the Market Director.
  • Communicate and coordinate appropriate air or ground transportation to and from receiving civilian or military facilities.
  • Assist the referring physicians with selecting the most appropriate transport option based on patient condition.
  • Communicate via radio with inbound aircraft to coordinate arrival time and arrival requirements.
  • Prepare transfer documents to include transport carrier forms, compact disks (CDs) of radiology reports and imaging, patient transfer packets, and transfer checklists. In preparing transfer documents, print, copy, and fax patient records.
  • Provide NMCCL and receiving facility printed resources to patients and family members prior to patient transfer to include information on NMCCL services and NMCCL point of contacts.
  • Communicate daily with external facilities on clinical status and progress of patients transferred from NMCCL. Collaborate daily with Utilization Management (UM) to determine if patients hospitalized in the regional civilian medical facilities are able to return to NMCCL for inpatient services.
  • Update informatics and documentation systems designed to record all patient movement from NMCCL to external facilities.
  • Prepare daily and ad hoc status reports that will be presented to MTF leadership for daily briefings.
  • Assist Registered Nurse - Case Managers in facilitating the referral process and coordinating patient appointments as appropriate.
  • Assist with completion of patient assessment questionnaires, patient satisfaction survey instruments, and other reports and tools used in the Transfer Center process.
  • Initiate, perform and complete assigned duties in providing care to variable patient populations.
  • Complete assigned patient care based on the patients’ conditions, use judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognize, report and record relevant patient information.
  • Observe, identify and respond to the patient’s needs for care, including medication, equipment-assisted care and patient/family education.
  • Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and perform treatments according to established policies/procedures.
  • Recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed.
  • Perform support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient’s condition before, during, and following the procedure.
  • Orient peers and ancillary personnel to unit policies and procedures, skills, and standards of practice.
  • Integrate healthy work environment principles that result in improved patient healthcare delivery, education of patients and their families and employee satisfaction.

  • Education/Certifications:
  • Education: Graduation from a State Board of Nursing approved LPN/LVN training program or completion of equivalent military training that permits sitting for the State licensure examination.
  • Licensure: Current, full, active and unrestricted license as a practical or vocational nurse.
  • Basic Life Support (BLS) Certification
  • Required Knowledge/Experience:
  • Experience: One year of experience within the last two years as a practical or vocational nurse.
  • Work Hours:
  • The Transfer Center operates 24 hours a day/7 days a week to include weekends and holidays.
  • Required to work every other weekend.
  • The scheduled hours shall be between 0600 and 1800 or 1800 and 0600, with 8.5-to-12.5-hour shift to include a 30-minute uncompensated meal break.
  • For each two-week period, the schedule to work will be 3 days one week (3 12.5-hour shifts) and 4 days the other week (3 12.5-hour shifts and 1 8.5-hour shift) and shall not exceed 80 hours worked.
  • The government will provide the schedule each month.

Pay: $32.00 - $35.20 per hour
