Licensed Social Worker

Overview
Join our dedicated healthcare team as a Licensed Social Worker, where you will play a vital role in providing compassionate, comprehensive social services to diverse patient populations. This dynamic position offers an exciting opportunity to make a meaningful difference in individuals’ lives through clinical counseling, case management, and community outreach. You will work collaboratively with multidisciplinary teams to support patients’ mental health, behavioral needs, and overall well-being across various settings including inpatient facilities, outpatient clinics, hospice care, and assisted living communities. Your expertise will help foster positive outcomes and empower patients on their journey to recovery and stability.

Duties

  • Conduct thorough patient assessments to evaluate mental health status, social needs, and risk factors using clinical tools and diagnostic evaluation methods.
  • Develop personalized treatment plans incorporating behavioral therapy, motivational interviewing, and psychotherapy techniques tailored to individual needs.
  • Provide individual and group counseling sessions focusing on mental health counseling, addiction counseling, PTSD care, dementia care, autism spectrum support, and developmental disabilities.
  • Coordinate case management activities such as intake procedures, medical documentation, data collection, and maintaining accurate medical records in compliance with HIPAA regulations.
  • Facilitate crisis intervention and crisis management strategies to ensure patient safety during emergencies or behavioral escalations.
  • Collaborate with healthcare professionals in hospital settings, nursing homes, assisted living facilities, and outpatient clinics to deliver integrated patient care utilizing EMR systems and ICD coding for accurate documentation.
  • Support families through child & family counseling services and assist with discharge planning for post-acute care or hospice patients.

Skills

  • Extensive experience in clinical research related to behavioral health or social work practices.
  • Proficiency in medical terminology, CPT coding, ICD-9/10 coding systems, and working knowledge of HIPAA standards for protected health information.
  • Strong background in working with individuals with disabilities, including Alzheimer’s disease, dementia, autism spectrum disorder, and developmental disabilities.
  • Skilled in crisis intervention techniques and crisis management strategies across inpatient, outpatient, home & community care environments.
  • Expertise in behavioral therapy modalities such as cognitive behavioral therapy (CBT), applied behavior analysis (ABA), and motivational interviewing.
  • Ability to utilize EMR systems effectively for documentation of patient care activities and medical records management.
  • Experience working with elderly populations in geriatrics or senior care settings; familiarity with hospice care protocols is a plus.

Pay: $34.13 - $41.10 per hour
