Job Overview
We are seeking a dedicated and compassionate Licensed Social Worker to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments to evaluate social, emotional, and environmental factors affecting health and well-being
- Provide individual and group psychotherapy sessions utilizing behavioral therapy, cognitive behavioral therapy (CBT), motivational interviewing, and other evidence-based approaches
- Assist with intake procedures, documentation, and medical records management in compliance with HIPAA regulations
- Develop personalized care plans that incorporate social work best practices for patients with mental health conditions, developmental disabilities, dementia, or post-acute care needs
- Collaborate with multidisciplinary teams including medical staff, therapists, and community resources to coordinate comprehensive patient care
- Facilitate crisis intervention and manage urgent situations with professionalism and sensitivity
- Support patients through discharge planning, home & community care arrangements, hospice services, or assisted living placements
Requirements
- Valid licensure as a Licensed Social Worker (LSW or LCSW) in the relevant state or jurisdiction
- Proven experience in hospital settings or outpatient clinics; experience with inpatient or nursing home environments is highly desirable
- Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and EMR systems for accurate documentation and data collection
- Expertise in child & family counseling, behavioral health interventions, addiction counseling, Alzheimer's care, PTSD care, dementia care, autism spectrum disorder support, and geriatrics is preferred
- Familiarity with crisis management techniques and working with individuals with disabilities or developmental delays
- Excellent communication skills for conducting diagnostic evaluations, case management activities, and working with diverse populations including children and seniors
- Ability to handle sensitive situations with empathy while maintaining compliance with HIPAA standards
