Responsibilities:

• Assessment and Intervention: Conduct thorough assessments of patients' and residents' social, emotional, and psychological needs, developing individualized plans to address these needs and promote overall well-being.
• Case Management: Coordinate with healthcare professionals to ensure seamless care transitions, facilitate communication among healthcare providers, and ensure compliance with regulatory requirements.
• Counseling and Support: Provide counseling and support to patients and residents to address grief, anxiety, depression, and other mental health concerns, as well as provide education and resources for caregivers and families.
• Documentation and Record Keeping: Maintain accurate and timely records of patient and resident interactions, assessments, and interventions, adhering to organizational standards and regulatory requirements.
• Collaboration and Communication: Work effectively with healthcare teams, including doctors, nurses, therapists, and other healthcare professionals, to ensure comprehensive care and support.
• Professional Development: Participate in ongoing education and training to stay current with industry developments, best practices, and organizational standards.
