Job Summary
We are seeking a dedicated and compassionate Licensed Social Worker (LSW) to join our dynamic healthcare team. Duties
- Conduct thorough patient assessments to identify social, emotional, and environmental needs using clinical counseling and diagnostic evaluation techniques.
- Develop tailored care plans incorporating behavioral health strategies such as cognitive behavioral therapy (CBT), motivational interviewing, and group therapy sessions.
- Coordinate with multidisciplinary teams to facilitate case management, discharge planning, and post-acute care transitions.
- Provide individual, family, and group counseling focused on mental health issues including PTSD, dementia care, Alzheimer's care, autism spectrum disorders, and disabilities.
- Document all patient interactions accurately in electronic medical records (EMR systems), ensuring compliance with HIPAA regulations and medical documentation standards.
- Assist with crisis intervention efforts and manage urgent situations through crisis management protocols.
- Support patients in navigating healthcare systems by explaining medical terminology, assisting with intake procedures, and collecting relevant data for clinical research or hospital records.
- Engage in community outreach initiatives to promote mental health awareness and social work services within diverse populations including seniors in assisted living or nursing homes.
Requirements
- Valid licensure as a Licensed Social Worker (LSW) with state-specific credentials required for practice.
- Proven experience working in hospital settings or inpatient units preferred; experience in hospice care or senior care is highly desirable.
- Strong knowledge of medical terminology, ICD coding (ICD-9/10), CPT coding, and familiarity with EMR systems for efficient documentation.
- Expertise in child & family counseling, behavioral therapy techniques such as applied behavior analysis (ABA), and working with individuals with developmental disabilities or autism spectrum disorders.
- Excellent skills in patient assessment, crisis intervention, case management, and clinical counseling methods including psychotherapy and motivational interviewing.
- Ability to handle sensitive situations involving mental health conditions like PTSD or dementia with compassion and professionalism.
- Knowledge of HIPAA regulations to ensure confidentiality of medical records while collecting data for research or clinical purposes.
- Strong communication skills for collaborating with healthcare teams and engaging effectively with patients from diverse backgrounds across outpatient or inpatient environments.
