Job Summary:

ATC Healthcare Services is seeking a compassionate and dedicated Licensed Social Worker (LSW) or Certified Social Worker (CSW) to work in a Day Care Center setting. The Social Worker will provide emotional support, case management, counseling, and resource coordination for participants and their families. The ideal candidate will have experience working with children, adults, seniors, or individuals requiring social services support.

Active LSW or CSW license is mandatory.

Essential Responsibilities:

• Conduct psychosocial assessments and evaluate participant needs.
• Provide counseling, emotional support, and crisis intervention services.
• Develop individualized care and service plans.
• Coordinate community resources and support services.
• Educate participants and families regarding available programs and services.
• Collaborate with nurses, therapists, caregivers, and interdisciplinary teams.
• Maintain accurate documentation and case records.
• Monitor participant progress and recommend appropriate interventions.
• Advocate for participant rights and well-being.
• Participate in care conferences and family meetings.

Qualifications:

• Bachelor’s or Master’s Degree in Social Work (BSW/MSW).
• Active CSW or LSW license is required and mandatory.
• Minimum 1 year of social work experience preferred.
• Experience in day care centers, community programs, schools, healthcare facilities, or behavioral health settings preferred.
• Strong communication, counseling, and case management skills.
• Ability to work independently and as part of a multidisciplinary team.

Preferred Experience:

• Experience working with children, seniors, or special needs populations.
• Knowledge of community resources and social service programs.
• Experience with documentation and care planning.

Pay: $25.00 - $35.00 per hour
