Job Title: SFC Care Associate (Licensed Social Worker)Role Overview:

The LSW serves as the primary liaison and coach for the family caregiver. Your goal is to ensure the caregiver is supported, the home environment remains stable, and the individual’s Person-Centered Service Plan (PCSP) is being followed.Key Responsibilities:
- Caregiver Coaching: Provide ongoing education and "care coaching" to the family caregiver on topics like stress management, community resources, and behavioral health support. [1, 5]
- Monthly Home Visits: Conduct at least one face-to-face home visit every 30 days to monitor the health, safety, and welfare of the individual. [5]
- Electronic Visit Verification (EVV) Oversight: Review the caregiver’s daily electronic notes and documentation to ensure compliance with Medicaid billing requirements. [3, 5]
- Initial Assessment: Assist the RN in the initial 30-day assessment to determine if the SFC placement is appropriate and if the caregiver can meet the individual's needs. [5]
- Service Coordination: Collaborate with the individual’s waiver case manager to update the service plan if the individual's needs change. [4, 5]
- Crisis Intervention: Act as the first point of contact for the caregiver if issues arise regarding household stability or caregiver burnout.
Required Qualifications:
- Licensure: Active Ohio Licensed Social Worker (LSW) or Licensed Independent Social Worker (LISW) in good standing. [5]
- Experience: Minimum of one year of experience in home and community-based services (HCBS) or working with the elderly/disabled population.
- Knowledge: Familiarity with OAC 5160-44-33 and Ohio’s Medicaid waiver programs (Ohio Home Care, PASSPORT, or MyCare).

- Strong expertise in crisis management and crisis intervention strategies.
- Proven experience in addiction counseling and behavioral health support.
- Excellent social work skills with a focus on client-centered approaches.
- Skilled in motivational interviewing to facilitate behavioral change.
- Supervisory experience with mentoring or overseeing less experienced staff or interns.
- Knowledge of child welfare systems, Section 8 housing programs, Medicare, and workforce development initiatives.
- Ability to handle sensitive situations involving childcare, mental health crises, or substance abuse with professionalism and compassion.
