Job Summary:

Homeside Rehab is seeking a compassionate and experienced Licensed Therapist (PT, OT, or SLP) to join our team in Menges Mills, PA. As a therapist with our organization, you will have the opportunity to provide exceptional rehabilitation services to patients in the comfort of their own homes. Responsibilities:

• Active Pennsylvania License (Required)

• Basic computer literacy skills

• 1 year of Home Health, Outpatient, or SNF experience preferred

• Ability to evaluate and develop a plan of care based on patients’ prior and current functionality

• Strong communication skills between office staff and therapists

• Ability to treat patients with therapeutic practices designed to relieve pain, increase mobility, restore normal function, and maintain maximum performance

• Maintain accurate and timely documentation of individualized care

• Collaborate with clinical staff to develop treatment plans that meet patient needs

• Adhere to industry standards and medical guidelines
