Overview
Join our dynamic mental health team as a Licensed Therapist / Social Worker (LISW | LPCC | LSW), where you will play a vital role in delivering comprehensive behavioral health services to diverse populations. You will provide evidence-based psychotherapy, conduct assessments, and develop personalized treatment plans within outpatient, inpatient, or telehealth settings. KEY RESPONSIBILITIES
Clinical Services
- Conduct comprehensive biopsychosocial assessments for youth and families referred to AMAVI behavioral health services.
- Develop, implement, and review individualized service plans (ISPs) and individualized treatment plans (ITPs) in collaboration with the treatment team, youth, and families.
- Provide individual, family, and group therapy utilizing evidence-based, trauma-informed treatment modalities appropriate to the population served.
- Facilitate psychoeducation sessions with youth and caregivers as clinically indicated.
- Participate in multidisciplinary treatment team meetings, discharge planning conferences, and case consultations.
- Conduct ongoing risk assessments and implement crisis safety planning in accordance with AMAVI protocols and Ohio law.
Discharge Planning & Aftercare
- Actively participate in discharge planning beginning at or within 72 hours of a youth’s admission, consistent with OAC 5122-30-32(B)(6).
- Coordinate with community-based providers, referring agencies, and natural supports to facilitate warm handoffs and continuity of care.
- Support development and documentation of aftercare plans, including six-month aftercare support as required for QRTP-served youth.
Documentation & Compliance
- Complete all clinical documentation accurately and within required timeframes, including assessments, progress notes, treatment plans, and discharge summaries.
- Maintain documentation in compliance with OAC Chapter 5122, AMAVI policies, and applicable accreditation standards.
- Participate in quality assurance activities and peer record reviews as requested by the LISW-S.
- Maintain confidentiality of all client records in compliance with HIPAA, 42 CFR Part 2 (if applicable), and Ohio law.
Supervision & Coordination
- Receive and actively engage in clinical supervision provided by the AMAVI LISW-S, including regular case consultations and competency reviews.
- Communicate significant clinical concerns, incidents, and critical events to the supervising LISW-S promptly.
- Collaborate with AMAVI’s clinical, nursing, and case management staff as part of an integrated treatment team.
- Attend required AMAVI trainings, staff meetings, and case conferences as specified in the contractor agreement.
MINIMUM QUALIFICATIONS
- Current, unrestricted Ohio license as a LISW (Licensed Independent Social Worker), LPCC (Licensed Professional Clinical Counselor), or LSW (Licensed Social Worker) in good standing with the Ohio Counselor, Social Worker and Marriage and Family Therapist Board.
- Master’s degree in Social Work (MSW) or other related degree from a CSWE-accredited program.
- Minimum of two (1) year of clinical experience in behavioral health, child welfare, or a related field.
- Experience working with youth, adolescents, and families in a residential, intensive outpatient, or community-based setting preferred.
- Demonstrated knowledge of trauma-informed care principles, evidence-based treatment modalities, and family-centered practice.
- Deliver individual, group, and family counseling utilizing modalities such as cognitive-behavioral therapy (CBT), psychodynamic therapy, applied behavior analysis (ABA), and crisis intervention strategies.
- Excellent communication skills with the ability to work effectively in a team-oriented setting while demonstrating compassion and professionalism.

- Familiarity with Ohio Administrative Code Chapter 5122 and QRTP standards preferred.
- Strong clinical documentation skills and experience with electronic health records (EHR).
- Ability to pass a background check in accordance with OAC 5122-30-31.
Pay: $25.00 - $35.00 per hour
