Job Summary
We are seeking a dedicated and compassionate Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. Duties

  • Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories using EMR (Electronic Medical Record) systems such as Cerner or eClinicalWorks.
  • Administer medications and injections accurately following prescribed protocols, including IV infusions and medication management for pain control or chronic conditions.
  • Assist with complex procedures such as catheterization, tube feeding, wound suturing, and sterile processing techniques to ensure patient safety and comfort.
  • Provide specialized care for patients with Alzheimer’s disease, dementia, or memory impairments through tailored interventions and behavioral health support.
  • Manage acute pain cases and participate in acute care settings such as ICU (Intensive Care Unit), Level I Trauma Centers, or emergency rooms by supporting airway management, ventilator operation, and trauma triage.
  • Conduct diagnostic evaluations including specimen collection/processing, phlebotomy (venipuncture), and basic medical imaging assistance when needed.
  • Support discharge planning and post-acute care coordination to facilitate smooth transitions from hospital to home or community-based settings.
  • Maintain meticulous medical documentation in compliance with HIPAA regulations while ensuring accuracy in ICD-9/ICD-10 coding for diagnoses and procedures.
  • Collaborate with multidisciplinary teams on case management efforts focusing on geriatrics, pediatric care (including neonatal ventilator management), or home & community health initiatives.
  • Educate patients and families on health coaching topics such as nutrition, medication adherence, disease management (e.g., infectious diseases), and preventive care strategies.
  • Uphold infection control standards through aseptic techniques and proper sterilization procedures during all patient interactions.

Experience

  • Valid LVN license with current state certification required; additional certifications in Basic Life Support (BLS) are mandatory.
  • Proven experience working in diverse healthcare environments such as hospitals (including trauma centers), nursing homes, hospice facilities, or outpatient clinics.
  • Demonstrated expertise in critical care areas like ICU or emergency medicine with familiarity in trauma triage protocols and acute pain management strategies.
  • Prior experience with EMR/EHR systems such as Cerner or Athenahealth to ensure seamless documentation and patient record management.
  • Background working with special populations including individuals with developmental disabilities, behavioral health needs, or those requiring memory/dementia care is highly desirable.
  • Knowledge of ICD coding (ICD-9/ICD-10), CPT coding for procedures like suturing or spinal taps, and familiarity with medical terminology enhances your ability to deliver precise patient care.
  • Experience administering injections—including botulinum toxin treatments—and performing venipuncture or catheter care confidently is essential.
  • Ability to work effectively in fast-paced settings such as urgent care clinics or hospital medicine units while maintaining attention to detail and patient safety standards.
