Licensed Vocational Nurse

<Job Summary>
Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) for IN HOME SHIFT CARE for a 9 year old kid, where your expertise will directly impact patient care and well-being. In this vital role, you will provide compassionate nursing services across various settings, including long-term care facilities, hospitals, outpatient clinics, and community health environments. Your responsibilities will include performing patient assessments, administering medications and treatments, and collaborating with multidisciplinary teams to ensure optimal health outcomes. <Responsibilities>

  • Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
  • Administer medications, injections, IV infusions, and other treatments following established protocols and safety standards.
  • Manage complex cases involving acute pain management, dementia care, Alzheimer's care, and memory care.
  • Operate electronic health record (EHR) systems such as Epic, Cerner, or Athenahealth to document patient information accurately and efficiently.
  • Assist with specialized procedures including catheterization, tube feeding, venipuncture, phlebotomy, sterile processing, and wound suturing.
  • Provide triage services by evaluating patient symptoms to determine urgency levels for emergency room or urgent care settings.
  • Collaborate with physicians and other healthcare professionals in discharge planning, case management, and post-acute care coordination.
  • Support patients with developmental disabilities or those requiring behavioral health interventions in both inpatient and community settings.
  • Ensure compliance with HIPAA regulations and maintain the highest standards of infection control and patient safety.

<Skills>

  • Proven experience working with EMR/EHR systems such as Epic, Cerner, or Athenahealth for medical documentation.
  • Knowledge of ICD-9/ICD-10 coding for accurate medical records billing and documentation.
  • Skilled in IV insertion, medication administration—including injections—and venipuncture techniques.
  • Familiarity with diverse clinical environments such as long-term care facilities, assisted living communities, hospice care, outpatient clinics, and pediatric or neonatal units.
  • Expertise in managing complex cases involving dementia/dementia-related conditions like Alzheimer’s disease or memory impairments.
  • Ability to perform physical examinations and diagnostic evaluations confidently while demonstrating physiology knowledge and anatomy expertise.
  • Experience in pain management strategies for acute or chronic conditions across various patient populations.
  • Competence in handling specialized procedures such as spinal taps, gastric lavage, dialysis support, or cardiac catheterization when applicable.
  • Excellent communication skills for effective patient assessment and health coaching across diverse age groups including children and seniors.
