Job Overview
We are seeking a dedicated and compassionate LVN to join our dynamic Adult Day Care Team. In this vital role, you will provide exceptional patient care across various settings, including acute care, long-term care, outpatient clinics, and community health environments. Responsibilities
- Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to determine individual care needs.
- Administer medications, injections, IV infusions, and treatments such as botulinum toxin therapy with precision and adherence to safety protocols.
- Utilize electronic health record (EHR) systems like Epic, Cerner, or Athenahealth to document patient information accurately and efficiently.
- Assist with complex procedures including catheterizations, suturing, tube feeding management, and ventilator care in various clinical settings such as ICU or surgical units.
- Coordinate patient care plans through case management strategies while ensuring compliance with HIPAA regulations and NCQA standards.
- Conduct diagnostic evaluations such as laboratory specimen collection/processing, ICD coding (ICD-9/ICD-10), and medical imaging when necessary.
- Support patients across diverse populations—including pediatrics, geriatrics, behavioral health, hospice & palliative medicine—delivering compassionate care tailored to individual needs.
Skills
- Proven experience in critical care environments such as Adult Day Care, ICU or Level I Trauma Centers; familiarity with trauma medicine and acute pain management is highly desirable.
- Strong knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques.
- Skilled in the use of EMR/EHR systems like Epic or Cerner for accurate medical documentation and patient records management.
- Expertise in performing procedures including IV insertion, phlebotomy, sterile technique, airway management, and ostomy care.
- Ability to work efficiently in fast-paced settings such as emergency rooms or urgent care clinics while maintaining high standards of patient safety and quality care.
- Experience working with individuals with disabilities or developmental delays; familiarity with memory care or dementia/dementia-related conditions is a plus.
