Job Summary
We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our dynamic healthcare team. In this vital role, you will provide compassionate patient care i our 6 bed congregate living facility. Your expertise will help ensure optimal health outcomes by performing thorough assessments, administering medications and treatments, and collaborating with multidisciplinary teams. Duties
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans.
- Administer medications, injections, IV infusions, and treatments following physician orders while adhering to safety protocols and HIPAA regulations.
- Manage complex care needs such as trach/vent (certification provided by facilty) wound care and acute pain management for diverse patient populations.
- Assist with specialized procedures including catheterization, tube feeding, venipuncture, sterile processing, and wound suturing.
- Utilize electronic health record POINT CLICK CARE to document patient information accurately and efficiently.
- Provide patient education on medication administration, disease management, and health coaching to promote wellness and independence.
- Collaborate with physicians, therapists, case managers, and other healthcare professionals to coordinate seamless patient care across settings such as inpatient units or outpatient clinics.
Skills
- Proven experience with Epic EMR systems or similar EHR platforms; familiarity with ICD-9/10 coding is highly desirable.
- Strong background in critical care environments including ICU or Level I Trauma Center experience; knowledge of trauma protocols is advantageous.
- Experience working with individuals with developmental disabilities or in specialized settings such as Alzheimer’s or dementia care.
- Skilled in performing triage assessments in urgent care or emergency room settings; capable of managing acute pain and infectious disease cases.
- Proficient in medical documentation, specimen collection/processing, phlebotomy, and injection administration techniques.
- Knowledge of anatomy and physiology combined with excellent patient assessment skills; ability to interpret diagnostic evaluations effectively.
- Ability to work efficiently within fast-paced environments like hospital medicine or post-acute care facilities while maintaining high standards of patient safety and confidentiality.
