Licensed Vocational Nurse - Charge Nurse 2pm-10pm

Join our dynamic healthcare team as a Licensed Vocational Nurse (LVN) - Charge Nurse, working the 2pm to 10pm shift. In this vital leadership role, you will oversee patient care delivery, coordinate nursing staff activities, and ensure the highest standards of quality and safety. Responsibilities

  • Supervise and coordinate nursing staff during your shift, ensuring seamless patient care delivery and adherence to clinical protocols
  • Conduct comprehensive patient assessments, vital signs monitoring, and physiological evaluations to inform care plans
  • Administer medications, including injections and IV infusions, following strict medical documentation and HIPAA privacy standards
  • Oversee medical procedures such as catheterization, sterile processing, wound suturing, and tube feeding with precision and aseptic technique
  • Collaborate with interdisciplinary teams on discharge planning, case management, and triage to optimize patient outcomes
  • Maintain accurate electronic health records (EHR systems like Epic or Cerner), ensuring compliance with ICD-9/ICD-10 coding standards for documentation accuracy
  • Provide education on health coaching, dementia care, Alzheimer's care, pain management, and other specialized areas for residents with complex needs

Skills

  • Proven experience in critical care environments such as ICU or Level I Trauma Centers; hospital medicine or emergency room experience highly valued
  • Strong knowledge of medical terminology, physiology, anatomy, and diagnostic evaluation techniques
  • Skilled in medical imaging procedures, phlebotomy, venipuncture, and specimen collection/processing
  • Proficiency with EMR/EHR systems including Epic, Cerner, Athenahealth or eClinicalWorks; familiarity with ICD coding (ICD-9/ICD-10) essential
  • Expertise in managing acute pain cases, infectious disease care, hospice services, and post-acute care settings
  • Ability to work effectively with individuals with disabilities or developmental delays; experience in geriatrics or assisted living preferred
  • Excellent patient assessment skills combined with compassionate communication and teamwork abilities

Pay: $27.00 - $29.00 per hour
