Licensed Vocational Nurse - Field Nurse

Job Summary
We are seeking a dedicated and energetic Licensed Vocational Nurse (LVN) to join our team as a Field Nurse, providing vital patient care in diverse community settings. In this role, you will deliver compassionate nursing services directly in patients’ homes, assisted living facilities, and other community environments. Your expertise will help improve health outcomes, promote independence, and support individuals with a variety of healthcare needs. Responsibilities

  • Conduct comprehensive patient assessments including vital signs, physical examinations, and health history documentation using EMR (Electronic Medical Records) systems such as Epic or Athenahealth.
  • Administer medications via injections, IV infusions, and other routes following strict protocols and HIPAA regulations to ensure patient safety.
  • Provide specialized care for individuals with complex conditions such as Alzheimer's disease, dementia, or memory impairments, including medication management and behavioral health support.
  • Assist with wound care, catheterization, tube feeding, and sterile procedures using aseptic techniques to promote healing and prevent infections.
  • Perform diagnostic evaluations such as specimen collection/processing and basic medical imaging procedures when necessary.
  • Educate patients and caregivers on health coaching topics including chronic disease management, medication adherence, nutrition, and discharge planning.
  • Collaborate with interdisciplinary teams on case management plans to optimize patient outcomes across settings like long-term care facilities, hospitals, or outpatient clinics.
  • Ensure all documentation complies with HIPAA standards and accurately reflects patient interactions for legal and billing purposes.

Experience

  • Valid LVN license with current state licensure required; additional certifications in Basic Life Support (BLS) preferred.
  • Proven experience working in diverse healthcare environments such as hospitals (including ICU or Level I Trauma Centers), nursing homes, hospice care, or home health settings.
  • Critical care experience including ICU or emergency room exposure is highly valued.
  • Familiarity with EMR/EHR systems like Epic, Cerner, or Athenahealth for efficient documentation and patient tracking.
  • Experience working with individuals with disabilities or special needs such as developmental disabilities or behavioral health challenges is advantageous.
  • Knowledge of ICD coding (ICD-9/10), CPT coding, and medical terminology to ensure accurate recordkeeping and billing practices.
  • Strong assessment skills in acute pain management, infectious disease care, geriatrics, pediatrics (including neonatal ventilator management), or post-acute care environments preferred.
