Job Summary
We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our healthcare team. The ideal candidate will possess a strong commitment to providing high-quality patient care in various settings, including hospitals, nursing homes, and assisted living facilities. The LVN will be responsible for delivering nursing care under the supervision of registered nurses and physicians, ensuring that patients receive the best possible support during their recovery and treatment.
Duties
- Provide direct patient care, including administering medications and treatments as prescribed.
- Conduct patient assessments and monitor vital signs to ensure optimal health outcomes.
- Assist with daily living activities for patients, including personal hygiene and mobility support.
- Collaborate with healthcare team members to develop and implement individualized care plans.
- Maintain accurate medical documentation in compliance with HIPAA regulations.
- Educate patients and their families about health management and disease prevention.
- Perform procedures such as IV insertion, catheterization, and specimen collection as needed.
- Participate in discharge planning and case management to facilitate smooth transitions of care.
- Provide support in specialized areas such as memory care, hospice care, and acute pain management.
Requirements
- Valid LVN license in the state of practice.
- Experience in critical care, long-term care, or assisted living environments is preferred.
- Familiarity with electronic medical records (EMR) systems such as Epic, Cerner, or eClinicalWorks is a plus.
- Knowledge of medical terminology, ICD coding (ICD-9 & ICD-10), and patient assessment techniques.
- Strong interpersonal skills with the ability to work effectively with diverse populations, including individuals with developmental disabilities and dementia care needs.
- Proficiency in basic life support (BLS) and experience with vital signs monitoring.
- Ability to perform procedures such as phlebotomy, tube feeding, and medication administration safely.
- Understanding of infection control practices and aseptic techniques.
