Licensed Vocational Nurse (LVN) – Home Health – Orange County, CA

Overview

Lynda’s Home Health Care is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) dedicated to delivering exceptional patient care in Orange County. Duties

• Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
• Administer medications via injections, IV infusions, and other routes following strict protocols to ensure safety and efficacy.
• Manage complex wound care procedures such as dressing changes, catheterizations, and sterile technique applications.
• Provide specialized care for patients with conditions like Alzheimer’s disease, dementia, or developmental disabilities, including memory care and behavioral health support.
• Coordinate discharge planning and collaborate with multidisciplinary teams using electronic health record (EHR) systems like Epic or Cerner to document medical records accurately.
• Conduct patient education on medication management, nutrition, pain management strategies, and health coaching tailored to individual needs.
• Assist with medical imaging procedures or diagnostic evaluations when required and ensure infection control standards are maintained at all times.

Requirements

• Valid LVN license in the state of California with current certification to practice nursing.
• Demonstrated experience in critical care settings such as ICU or trauma centers is highly preferred.
• Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for documentation and case management.
• Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and medication administration protocols.
• Experience working with diverse populations such as seniors in assisted living or long-term care environments; familiarity with hospice or palliative care is a plus.
• Ability to perform complex procedures including IV insertions, phlebotomy, venipuncture, tube feeding management, and ventilator support if applicable.
• Excellent patient assessment skills combined with compassionate communication tailored to individuals with cognitive impairments like Alzheimer’s or dementia.
• Knowledge of HIPAA regulations and infection control practices to ensure patient confidentiality and safety at all times.
