Overview
Lynda’s Home Health Care is seeking a compassionate and dependable Licensed Vocational Nurse (LVN) dedicated to delivering exceptional patient care in Orange County. This energetic role offers the opportunity to work closely with diverse patient populations, including seniors, individuals with disabilities, and those recovering from illness or injury. As an LVN in home health, you will play a vital part in promoting health, independence, and comfort for your patients while working within a supportive and innovative environment. Duties
- Perform comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine care needs.
- Administer medications via injections, IV infusions, and other routes following strict protocols to ensure safety and efficacy.
- Manage complex wound care procedures such as dressing changes, catheterizations, and sterile technique applications.
- Provide specialized care for patients with conditions like Alzheimer’s disease, dementia, or developmental disabilities, including memory care and behavioral health support.
- Coordinate discharge planning and collaborate with multidisciplinary teams using electronic health record (EHR) systems like Epic or Cerner to document medical records accurately.
- Conduct patient education on medication management, nutrition, pain management strategies, and health coaching tailored to individual needs.
- Assist with medical imaging procedures or diagnostic evaluations when required and ensure infection control standards are maintained at all times.
Requirements
- Valid LVN license in the state of California with current certification to practice nursing.
- Demonstrated experience in critical care settings such as ICU or trauma centers is highly preferred.
- Proficiency with EMR/EHR systems including Epic, Cerner, or Athenahealth for documentation and case management.
- Strong knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/10), CPT coding, and medication administration protocols.
- Experience working with diverse populations such as seniors in assisted living or long-term care environments; familiarity with hospice or palliative care is a plus.
- Ability to perform complex procedures including IV insertions, phlebotomy, venipuncture, tube feeding management, and ventilator support if applicable.
- Excellent patient assessment skills combined with compassionate communication tailored to individuals with cognitive impairments like Alzheimer’s or dementia.
- Knowledge of HIPAA regulations and infection control practices to ensure patient confidentiality and safety at all times.
