Licensed Vocational Nurse - Private Duty Nurse

Join our dedicated healthcare team as a Licensed Vocational Nurse (LVN) specializing in private duty nursing! In this vital role, you will provide personalized, high-quality nursing care directly in patients' homes or private settings, ensuring comfort, safety, and optimal health outcomes. Your expertise will support individuals with diverse medical needs, including those requiring complex care such as IV infusions, dementia care, and acute pain management. <Responsibilities>

  • Deliver comprehensive patient assessments, including vital signs, physical examinations, and physiological evaluations to determine individualized care plans.
  • Administer medications, injections, IV infusions, and other treatments following strict protocols and HIPAA regulations to ensure patient safety.
  • Manage specialized procedures such as catheterization, tube feeding, wound suturing, and sterile dressing changes with precision and aseptic technique.
  • Monitor patients’ conditions continuously, utilizing EMR (Electronic Medical Records) systems like Epic or Cerner to document medical records accurately and efficiently.
  • Collaborate with multidisciplinary teams for discharge planning, case management, and coordination of post-acute or hospice care services.
  • Provide education and health coaching to patients and families on disease management, medication administration, and lifestyle modifications.
  • Maintain infection control standards while working with individuals with disabilities or chronic conditions such as Alzheimer’s disease or dementia.

<Skills>

  • Proven experience in critical care environments such as ICU or Level I Trauma Centers, with familiarity in acute care procedures and trauma protocols.
  • Strong background working with diverse populations including pediatrics, geriatrics, neonatal ventilator management, or individuals with developmental disabilities.
  • Proficiency in medical documentation using EMR/EHR systems like Athenahealth or eClinicalWorks; knowledge of ICD-9/10 coding is a plus.
  • Skilled in patient assessment techniques including vital signs measurement, diagnostic evaluation, and physiology knowledge.
  • Expertise in medication administration including injections and IV insertion; experience with advanced procedures like spinal taps or cardiology catheterization is advantageous.
  • Excellent understanding of HIPAA compliance, infection control practices, and sterile processing techniques.

  • Schedule: Monday to Friday From 10PM TO 6AM - THIS IS FOR ONE PATIENT IN HOME CARE

Pay: $35.00 - $37.00 per hour
