Responsibilities
- Provide attentive patient observation to monitor health status and respond promptly to changes or emergencies.
- Assist with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using equipment such as Hoyer lifts.
- Administer medications accurately following medical instructions and document medication records in compliance with HIPAA regulations.
- Support clients with memory care needs, dementia, Alzheimer's disease, and other cognitive impairments through specialized care techniques.
- Perform vital signs assessments, catheter care, and hospice support as needed to ensure comfort and safety.
- Maintain detailed medical documentation using Electronic Health Record (EHR) systems and ensure all medical records are current and confidential.
- Assist with light housekeeping tasks such as cleaning, laundry, meal preparation, and ensuring a safe living environment.
- Collaborate with healthcare professionals by updating medical records and communicating patient progress effectively.
- Provide emotional support and companionship to promote mental well-being and social engagement.
- Follow all protocols related to home & community care standards, including working with individuals with disabilities or developmental challenges.
Requirements
- Proven experience in senior care, home health, assisted living, or long-term care environments.
- Knowledge of medical terminology, medication administration, CPR certification, and vital signs monitoring.
- Experience working with individuals with dementia, Alzheimer's disease, developmental disabilities or in hospice care settings.
- Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation.
- Ability to assist with ADLs using appropriate equipment like Hoyer lifts or other mobility aids.
- Strong patient observation skills combined with compassionate caregiving techniques.
- Excellent communication skills to coordinate effectively with healthcare teams while maintaining patient confidentiality per HIPAA guidelines.
- Flexibility for live-in employment arrangements; ability to adapt to varying schedules as needed.
- Physical stamina to perform caregiving duties including lifting, cleaning, meal prep, and mobility assistance.
