Responsibilities
- Deliver attentive patient observation to monitor health status and respond promptly to changes or emergencies
- Assist with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
- Administer medications accurately following medical instructions and document medication records in compliance with HIPAA regulations
- Support clients with memory care needs including Alzheimer’s and dementia care by implementing personalized routines and safety measures
- Perform vital signs checks and record medical data using Electronic Health Record (EHR) systems or Electronic Medical Records (EMR) systems to ensure precise documentation
- Provide assistance with catheter care, hospice support, and other specialized medical procedures under the supervision of healthcare professionals
- Prepare nutritious meals tailored to dietary restrictions while maintaining cleanliness and hygiene standards in the home environment
- Conduct light housekeeping tasks such as laundry, cleaning, and organizing to maintain a safe and comfortable living space
- Support clients during medical appointments by assisting with transportation and ensuring proper medical documentation is maintained
- Collaborate with healthcare teams to coordinate home & community care services that meet individual needs for long-term or short-term care
Experience
- Prior experience working in assisted living facilities, nursing homes, or home health environments is highly preferred
- Demonstrated expertise in patient care including dementia/dementia care, Alzheimer's care, hospice support, and working with individuals with developmental disabilities or disabilities
- Familiarity with medical terminology, medication administration protocols, and vital signs monitoring
- Hands-on experience operating medical equipment such as Hoyer lifts or catheters is advantageous
- Knowledge of HIPAA regulations ensuring confidentiality of patient records and medical information
- Proven ability to work independently in a live-in employment setting while maintaining professional boundaries and compassion
- Strong skills in caregiving tasks including meal preparation, cleaning, laundry, and general household management
