Long Term Care Social Worker Assistant

Job Summary
We are seeking a compassionate and dedicated Long Term Care Social Worker to join our team and make a meaningful difference in the lives of individuals requiring extended care. In this vital role, you will provide comprehensive social services, support behavioral health needs, and assist clients in navigating complex healthcare systems. Responsibilities

  • Conduct thorough assessments of residents’ social, emotional, and behavioral health needs to develop personalized care plans.
  • Provide crisis intervention and crisis management support to residents experiencing emotional or behavioral distress.
  • Facilitate access to community resources such as Medicaid, Medicare, Section 8 housing, and child welfare programs to support residents’ needs.
  • Offer addiction counseling and behavioral health services to promote recovery and mental wellness.
  • Collaborate with multidisciplinary teams including healthcare providers, therapists, and family members to coordinate comprehensive care.
  • Supervise and mentor junior staff or interns, ensuring adherence to best practices in social work and client engagement.
  • Assist residents with navigating complex systems related to workforce development, childcare support, and housing stability.
  • Utilize motivational interviewing techniques to encourage positive behavioral changes and foster resident independence.

Skills

  • Strong expertise in crisis intervention and crisis management strategies.
  • Proficiency in addiction counseling and behavioral health interventions.
  • Knowledge of social work principles, including client advocacy and case management.
  • Ability to handle sensitive situations
  • Excellent communication skills for engaging with diverse populations and multidisciplinary teams.
  • Familiarity with Medicare regulations, Medicaid services, and community resource navigation.
