Job Summary
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our dynamic healthcare team. In this vital role, you will provide high-quality patient care across a variety of settings, including long-term care facilities, assisted living communities, hospitals, and outpatient clinics. Your expertise will support patient recovery, comfort, and well-being through skilled nursing interventions, assessments, and compassionate communication. Duties
- Perform comprehensive patient assessments including vital signs, physical examinations, and physiological evaluations to inform care plans.
- Administer medications and injections accurately following physician orders, including IV infusions and advanced medication protocols.
- Assist with complex procedures such as catheterization, wound suturing, tube feeding, and sterile processing techniques.
- Manage patient documentation diligently using EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems like Cerner or Athenahealth to ensure compliance with HIPAA standards.
- Provide specialized care for patients with conditions such as Alzheimer's disease, dementia, infectious diseases, or those requiring hospice or palliative care.
- Support acute pain management strategies and participate in triage activities for urgent medical needs in emergency or urgent care settings.
- Collaborate with multidisciplinary teams including physicians, case managers, physical therapists, and social workers to coordinate comprehensive patient care plans.
Qualifications
- Valid LPN license with current state licensure required; additional certifications such as Basic Life Support (BLS) preferred.
- Demonstrated experience working with diverse populations including geriatrics, pediatrics, neonatal ventilator patients, or individuals with developmental disabilities.
- Proven expertise in critical care environments such as ICU or Level I Trauma Centers; hospital medicine experience highly valued.
- Familiarity with ICD-9/ICD-10 coding systems for accurate medical documentation and billing processes.
- Experience performing phlebotomy, venipuncture, specimen collection/processing, and administering injections with aseptic technique.
- Knowledge of anatomy and physiology principles to support accurate patient assessment and diagnostic evaluation.
- Strong communication skills to effectively educate patients on health coaching topics like nutrition or medication management while maintaining confidentiality under HIPAA regulations.
