This is a full time position.
 
Responsibilities
 
<ul>
<li>Must participate in after hours phone rotation.</li>
<li>Assist care manager with enrolled patients.</li>
<li>Provides direct and indirect patient care.</li>
</ul>
Qualifications
 
<ul>
<li>Current LPN license</li>
<li>3-5 years LPN experience preferred.</li>
<li>Effective written and verbal communication</li>
<li>US Citizen or Green Card holder</li>
</ul>
