What You’ll Do

As an ADvantage Case Manager, you will:

• Conduct assessments and develop individualized service plans

• Initiate and monitor care services to ensure member needs are met

• Set realistic, goal-oriented care objectives

• Provide resource referrals and ongoing support

• Maintain accurate, timely documentation

• Collaborate with providers, members, and families to ensure quality outcomes

No prior ADvantage Case Management experience? No problem — we provide training for the right candidate.

What We’re Looking For

• Active LPN license or Bachelor’s degree

• Minimum of 1 year of geriatric experience (nursing home, home health, or related setting)

• Strong organizational and time-management skills

• Ability to work independently

• Excellent computer and typing skills
