Job Description

Activa Home Health, a licensed Medicare-certified home health agency, is seeking experienced Field Licensed Practical Nurses (LPNs) for hourly shifts throughout Broward County.

Key Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with the Plan of Care

• Monitor patient condition and report changes to the supervising nurse or clinical team

• Administer medications and perform treatments within scope of practice

• Maintain accurate and timely clinical documentation

• Communicate effectively with patients, families, and interdisciplinary care team members

Qualifications

• Active Florida LPN license

• Prior home health experience preferred

• Strong clinical, communication, and documentation skills

• Ability to work independently in the field

• Reliable transportation and valid driver’s license

• Ability to pass background screening and drug testing
