Job Summary: LPNs work under the supervision of a Registered Nurse (RN) or physician, and act as first-line supervisor to resident aides and medication technicians on shift. Primary responsibilities include monitoring and assisting residents with medications, alerting relevant individuals of changes in resident conditions, and following up with all resident care issues while maintaining clinical quality assurance in accordance with New York State regulations and adherence to Longview's risk management practices. This role also assists residents with daily personal care and supports resident engagement in social and recreational activities which promote their physical and emotional health.

Strong communication skills, patience, and a compassionate approach are essential as this position works closely with residents, their families, and the healthcare team to deliver high-quality care.
Schedule Considerations: Consistent schedule, every other weekend required, holiday shifts as scheduled/needed (24/7/365)
Compensation: $28.00-34.00 per hour commensurate with experience and credentials.

Requirements:
Typical job duties and responsibilities include but are not limited to the following:

Oversee the work of resident services direct care staff, including resident aides and medication technicians, as a first-line supervisor;
-
Teach and guide team members to uphold clinical excellence and quality assurance;
-
Adhere to facility policies/procedures and scope of practice for LPNs in New York, ensuring compliance with all relevant regulations and guidelines;
-
Process daily paperwork including physician orders, appointment forms, medication and treatment records in MAR/TAR, and hospital/ER discharges;
-
Assist with admission paperwork and auditing medical/evaluation forms;
-
Complete 3122 forms, DME orders, and other relevant documentation timely and appropriately in resident individualized service plans;
-
Assist with quality assurance audits of residents who self-medicate including those who self-administer blood glucose monitoring and insulin pens;
-
Oversee and assist with resident personal care and daily living activities as indicated in their service plan;
-
Assist residents with medications given on shift;
-
Oversee shift change and participate, as necessary, in shift reporting;
-
Participate in treatment team meetings if working day shift;
-
Maintain timely communication with direct care staff, case managers, physicians and families regarding pertinent resident issues and changes in conditions;
-
Support resident participation in social and recreational activities;
-
Monitor and document residents' physical and emotional wellbeing, reporting any changes to the RN;
-
Participate in care plan meetings and contribute to the development/review of resident care plans;
-
Maintain a clean, safe, and comfortable living environment;
-
Collaborate with healthcare professionals to ensure comprehensive care;
-
Monitor nurse call console and provide necessary assistance in a timely fashion;
-
Complete rounds at regularly scheduled intervals to insure the safety of residents and the facility;
-
Report all accidents/injuries to supervisor immediately, documenting them on appropriate form(s) in accordance with organization risk management procedures;
-
Attend mandatory staff meetings and in-services as scheduled;
-
Maintain required job-related certifications specific to job description, if any;
-
Work independently, within a team, and with direct supervision;
-
Exhibit professionalism, honesty, and respect;

Maintain a positive attitude;

Maintain a professional relationship with all coworkers, supervisors, residents, and other stakeholders;

Fulfill emergency staffing needs, as necessary;

Qualifications:
-
Must be at least 18 years of age;
-
Must hold a valid LPN license in the state of New York and be in good standing;
-
Maintain current CPR/First Aide certification;
-
One (1) year of relevant experience or equivalent combination of education, training, and experience in a medical or healthcare setting;

Demonstrate strong leadership skills including good judgment, problem-solving, and decision-making capabilities;
-
MAT (medication administration training) or willingness to obtain;
-
Must be able to communicate clearly, concisely, and with empathy to build rapport and achieve high quality of care;

Possess excellent written and verbal communication skills;
-
Basic knowledge of resident rights;
-
Must complete basic First Aid, Heimlich, and Bloodborne Pathogen training within three (3) months of hire and maintain certifications.

Able to push, pull, lift, and carry trays of food, laundry bags, heavy boxes, and/or equipment weighing up to 50 pounds without restrictions;

Must be able to stand for long periods of time, frequently bending, stooping, and reaching;
-
Must be flexible in schedule for emergencies, have reliable transportation, and maintain a working phone number;
-
Satisfactory completion of post-offer, pre-employment physical required.
