Overview
We are seeking a motivated and energetic Part-Time Licensed Practical Nurse (LPN) to join our home health care team. In this role, you will provide vital nursing services to patients in their homes, promoting health, safety, and well-being. This position offers flexible hours and the opportunity to make a meaningful difference in patients’ lives through personalized care.

Responsibilities

• Perform comprehensive patient assessments, including vital signs, physical examinations, and medication administration.
• Deliver skilled nursing care such as wound dressing, IV infusion, catheter care, and tube feeding in accordance with physician orders.
• Manage patient documentation accurately using Electronic Health Record (EHR) systems like Epic or Cerner.
• Educate patients and their families on health management, medication use, and disease prevention strategies.
• Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients with complex needs.
• Assist with discharge planning and coordinate ongoing care for long-term or post-acute patients.
• Ensure infection control protocols are strictly followed to maintain a sterile environment.

Qualifications

• Valid LPN license in good standing; additional certifications in Basic Life Support (BLS) required.
• Prior experience in home health care or related settings such as hospitals, nursing homes, assisted living or hospice preferred.
• Knowledge of medical terminology, anatomy, physiology, and ICD coding systems (ICD-9/10).
• Experience with EMR/EHR systems such as Epic, Athenahealth or eClinicalWorks is highly desirable.
• Strong patient assessment skills with the ability to work independently and manage multiple cases effectively.
• Critical care experience or background working with individuals with developmental disabilities, dementia or memory care needs is a plus.
