Job Overview
The LPN plays a key role in coordinating care for a caseload of patients and ensuring it meets quality standards and utilization guidelines. The RN Case Manager assesses, plans, implements, monitors, and evaluates comprehensive care plans to ensure optimal patient outcomes. This position demands a compassionate, skilled nurse who thrives in a home-based environment and values patient education and family engagement.
What You’ll Do:
Schedule:
Full-time
Flexible schedule with a combination of remote and in-field responsibilities
Some in-office presence required weekly
Location:
You’re Perfect for the Position if you:
Are a graduate of an accredited LPN program
Hold current Georgia licensure and CPR certification
Have 2 years of nursing experience in hospice, or home health
Understand Medicare/Medicaid regulations and care coordination best practices
Can function independently in a community-based environment
Possess strong clinical, documentation, and time management skills
Are comfortable with EMR systems and standard office software
Have reliable transportation and are comfortable traveling to client homes
