Join our compassionate healthcare team as a Licensed Practical Nurse (LPN) specializing in hospice care. Duties

  • Deliver comprehensive patient assessments, including vital signs, symptom management, and physiological evaluations to inform personalized care plans.
  • Administer medications and treatments such as IV infusions, injections, and tube feedings while adhering to strict safety protocols and HIPAA regulations.
  • Manage complex symptom control including acute pain management, Alzheimer's and dementia care, and infectious disease considerations within hospice settings.
  • Coordinate discharge planning and collaborate with families and caregivers to ensure seamless transitions and ongoing support at home or in assisted living environments.
  • Document all patient interactions accurately using EMR (Electronic Medical Records) systems like Epic or Athenahealth; maintain detailed medical records consistent with NCQA standards.
  • Provide education on disease progression, medication administration, and safety measures such as catheter care, venipuncture, and sterile techniques.
  • Respond promptly to triage calls and urgent patient needs by assessing symptoms and providing appropriate guidance or escalation of care.

Experience

  • Valid LPN license with current state licensure required; additional certifications in Basic Life Support (BLS) strongly preferred.
  • Demonstrated experience in hospice or palliative care settings is highly desirable; experience working with individuals with disabilities or in long-term care environments is a plus.
  • Proven background in critical care areas such as ICU, emergency medicine, or trauma centers enhances your ability to manage complex cases effectively.
  • Familiarity with EMR/EHR systems like Cerner, eClinicalWorks, or similar platforms for accurate documentation and patient tracking.
  • Experience working with diverse populations including geriatrics, memory care patients, or those requiring specialized dementia or Alzheimer's care is advantageous.
  • Strong assessment skills combined with physiology knowledge enable precise patient evaluations; prior experience in home & community care or post-acute settings is beneficial. Pay: $35.96 - $50.00 per hour
