Responsibilities:

• Provide direct patient care to residents, including administering medications, performing treatments, and documenting patient progress
• Develop and implement individualized care plans in collaboration with the healthcare team
• Monitor and report changes in patient condition to the charge nurse or physician
• Maintain a clean, safe, and organized patient care environment
• Participate in quality improvement initiatives and contribute to the development of policies and procedures
• Collaborate with other departments to ensure seamless delivery of care

Requirements:

• Current licensure as a Licensed Practical Nurse (LPN) in the state of Ohio
• High school diploma or equivalent required; associate's or bachelor's degree in nursing or a related field preferred
