Job Overview
We are seeking a dedicated Licensed Practical Nurse (LPN) specializing in Skilled Nursing Facility (SNF) care to join our dynamic healthcare team. The ideal candidate will possess a strong foundation in patient care, with expertise in managing complex medical needs within a long-term care environment. As an LPN SNF, you will play a vital role in providing compassionate, high-quality nursing services to residents, ensuring their safety, comfort, and well-being. Your responsibilities will include performing clinical assessments, administering medications, and collaborating with interdisciplinary teams to deliver comprehensive care.
Duties
- Provide direct patient care by monitoring vital signs, administering medications, and performing routine clinical procedures such as IV infusions and catheter care.
- Conduct thorough patient assessments to identify health needs and assist with developing individualized care plans.
- Assist with activities of daily living (ADLs), including mobility support, hygiene, and nutrition management.
- Document patient information accurately using Electronic Health Record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations.
- Collaborate with physicians, therapists, and other healthcare professionals to coordinate treatment plans and ensure continuity of care.
- Support residents with specialized needs such as Alzheimer's and dementia care, memory care programs, and behavioral health interventions.
- Respond promptly to emergency situations, including acute pain management and airway management protocols in accordance with established procedures.
- Participate in discharge planning and family education to facilitate smooth transitions of care within the long-term care setting.
Requirements
- Valid LPN license in good standing; previous experience working in a Skilled Nursing Facility or long-term care setting is highly preferred.
- Demonstrated proficiency with EMR/EHR systems such as Epic, Athenahealth, or eClinicalWorks.
- Experience with critical care environments including hospital medicine, ICU, or Level I Trauma Centers is advantageous.
- Knowledge of ICD-9/ICD-10 coding for accurate documentation and billing processes.
- Familiarity with specialized areas such as hospice care, infectious disease management, and acute pain control.
- Strong understanding of physiology, anatomy, pharmacology, and medical terminology relevant to geriatric and complex patient populations.
- Ability to perform phlebotomy, injections administration, specimen collection/processing, and sterile techniques as required.
- Excellent communication skills for effective patient assessment and team collaboration; compassionate approach toward working with individuals with disabilities or cognitive impairments.
