Job Summary We are seeking a dedicated and skilled Licensed Practical Nurse (LPN) to provide high-quality, compassionate care to patients in a skilled nursing home environment. This full-time day shift position works from 7 AM to 7 PM and requires specialized experience in tracheostomy care. The ideal candidate will be detail-oriented, a strong communicator, and committed to ensuring the highest standards of patient safety and well-being under the supervision of a Registered Nurse or physician.
Key Responsibilities
- Monitor patient vital signs and overall health status, reporting any significant changes promptly to the supervising RN or physician.
- Provide specialized direct patient care, including tracheostomy care, suctioning, and potentially managing ventilators or G-tubes if applicable to the patient's needs.
- Administer medications and treatments as prescribed by the physician, adhering strictly to all safety standards and facility policies.
- Perform wound care and change dressings as needed.
- Assist patients with activities of daily living (ADLs), including bathing, dressing, and eating, ensuring comfort and dignity.
- Maintain accurate, thorough, and timely patient health records and clinical documentation in our electronic medical records (EMR) system.
- Educate patients and their families on care plans, medications, and disease management to ensure a smooth continuum of care.
- Respond quickly and effectively to changes in patient condition or emergency situations.
- Collaborate effectively with the interdisciplinary healthcare team to ensure coordinated and comprehensive patient care.
Qualifications & Skills
- Current and valid LPN license in the state of Mississippi (MS) is a must.
- Proven experience as an LPN, preferably in a [home health, acute care, long-term care] setting.
- Specific experience with tracheostomy care is required. (Experience with ventilators/G-tubes is a strong plus).
- Current Basic Life Support (BLS) or CPR certification from the American Heart Association or Red Cross.
- Strong clinical skills, attention to detail, and sound clinical judgment.
- Excellent communication, interpersonal skills, and a compassionate, patient-focused approach to care.
- Ability to manage time effectively, prioritize workload, and maintain composure in a fast-paced environment.
