Role Overview

As a Patient Care Coordinator, you’ll guide patients through their health journeys via monthly phone outreach, individualized care plans, and coaching on wellness, nutrition, and treatment goals.

Requirements

• Active Compact LPN license.
• 2+ years clinical experience (care coordination preferred).
• Comfortable with EMRs, Microsoft Office, and other digital tools.
• Strong communication and problem-solving skills.
• Must use a desktop or laptop for assessments and work (tablets/phones not compatible).
  
  

Schedule & Pay

• Full-time, 40 hrs/week | Monday-Friday, 8 AM-6 PM (patient’s time zone).
• Orientation/Training (first 2 months): $20/hr.
• Monthly bonus potential up to $525.
• Referral bonuses up to $1,000.
