LVN Case Manager- Home Health

Overview

The LVN Case Manager provides clinical oversight and coordination of all home health patient cases. This is a non-field, in-office role responsible for ensuring that each patient’s Plan of Care is appropriate, effective, and compliant with regulatory and quality standards. The LVN Case Manager reviews and approves clinical documentation, monitors patient outcomes, supports field clinicians, and serves as a central liaison between patients, physicians, and the interdisciplinary team.

Duties

  • Establish, monitor and update individualized care plans (Plan of Care / POC) in collaboration with the interdisciplinary team, in accordance with physician orders, and agency policy.
  • Coordinate care and services: collaborate with field nurses, home health aides, therapists, social workers, physicians, other healthcare providers and community resources to ensure seamless delivery of care and effective transitions.
  • Ensure all documentation (OASIS, nursing assessments, progress notes, Plan of Care reviews, physician communications) is accurate, timely and compliant with regulatory requirements and agency standards.
  • Coordinate needs of new referrals, resumption of care, recertification and discharges. Input 30 and 60 day summaries along with LVN and HHA supervisory notes within Carevoyant.
  • Serve as patient advocate: educate patients and families/caregivers about the care plan, home health services, self-management, equipment, community resources and discharge planning.
  • Participate in quality improvement initiatives, risk management, clinical audits, and maintain current knowledge of best practices, home health regulations and industry changes.

Required Qualifications

  • Active, unrestricted California Licensed Vocational Nurse (LVN) license.
  • Minimum of 2 years of clinical nursing experience as a Case Manager for a home health agency.
  • Strong assessment, critical thinking, care-planning and documentation skills.
  • Excellent interpersonal, communication, organizational and time-management skills; ability to lead and facilitate weekly case conferences.
  • In-depth knowledge of OASIS, CMS, and home health regulatory requirements.
  • BLS
  • Ability to work independently and collaborate effectively with a multidisciplinary team.

Pay: $28.00 - $31.00 per hour

Expected hours: 20.0 per week
