We are currently seeking an LVN Clinical Coordinator to join our Hospice team. This role offers the opportunity to build meaningful relationships with patients and make a tremendous impact in the community—while receiving competitive pay and excellent benefits.
Job Summary
We are seeking a compassionate and experienced LVN Clinical Coordinator to support hospice referral, intake, and care coordination processes. This role ensures high-quality patient care, regulatory compliance, and seamless transitions to hospice services in accordance with state, federal, and Joint Commission guidelines.
Key Responsibilities
Manage daily hospice referrals and intake operations, including policy oversight
Coordinate timely hospice admissions and transitions to home care
Review clinical documentation to determine hospice eligibility
Build and maintain relationships with physicians, hospitals, and referral sources
Track and report clinical metrics (admissions, discharges, GIP, respite, deaths)
Participate in interdisciplinary case conferences and QAPI initiatives
Communicate hospice services to patients, families, and caregivers
Coordinate plans of care with physicians and care teams
Provide support for intake, scheduling, urgent needs, and medication orders
Assist with staff education, orientation, and policy development
Qualifications
Active License Vocational Nurse (LVN) license in California
Minimum 3 years of nursing experience (hospice or home health preferred)
Strong communication, organization, and clinical documentation skills
Current CPR certification
Knowledge of hospice philosophy and end-of-life care
