Shift : Monday–Friday, 8:00 AM – 5:00 PM (5x8s) | No Call | Day Shift

Mile Rule: Must Live 50Miles outside of Clearlake

Job Summary: We are looking for an LVN Clinical Intake Coordinator, you will serve as a critical connection point between patients, families, physicians, and care teams ensuring seamless transitions into home health services and timely coordination of care.

Responsibilities

In this fast-paced coordination role, you will:

• Manage and coordinate incoming referrals, calls, and inquiries from patients, families, physicians, and healthcare partners

• Verify insurance coverage and ensure accurate intake processing

• Coordinate home health referrals and ensure timely patient onboarding

• Track hospitalized patients and facilitate smooth transitions into agency care

• Communicate effectively with nursing, therapy, social work, and physician teams

• Assist with scheduling and staffing coordination based on program needs

• Support operational workflows and ensure continuity of care across disciplines

• Serve as a working coordinator for intake processes and workflow efficiency

Experience/Qualifications

• Active LVN (Licensed Vocational Nurse) certification

• Minimum 1 year of healthcare experience

• High School Diploma or GED

Preferred :

• Associate’s or Technical Degree (or equivalent experience)

• 3+ years of healthcare experience, ideally in home health, intake, or care coordination
