Home Health LVN

Overview:
We are seeking a compassionate and Licensed Vocational Nurse LVN to join our Home Health team. As a Licensed Vocational Nurse - Home Health, you will provide comprehensive nursing care to patients in the comfort of their own homes. Duties:
- Perform physical examinations and assess patients' medical conditions
- Conduct diagnostic evaluations and interpret test results
- Administer medications and treatments as prescribed by physicians
- Utilize aseptic technique to prevent infection during procedures
- Maintain accurate and up-to-date medical records
- Collaborate with interdisciplinary healthcare team members to develop and implement patient care plans
- Provide wound care, including suturing when necessary
- Ensure compliance with coding and documentation guidelines
- Coordinate with nursing homes and other healthcare facilities for patient transfers or referrals
- Implement infection control measures to prevent the spread of diseases

Experience:
- Minimum of 2 years of experience as a LVN in a clinical setting, preferably in home health or a related field
- Strong knowledge of physiology and medical terminology
- Proficient in performing physical assessments and interpreting diagnostic tests
- Familiarity with coding and documentation requirements
- Experience with wound care, including suturing, is preferred
- Excellent communication and interpersonal skills

Requirements:
- Valid Licensed Vocational Nurse license in the state
- Bachelor's degree in Nursing (BSN) preferred but not required
- Current CPR certification
- Reliable transportation for home visits
